Meet Jennifer
Licensed Clinical Social Worker, Certified Case Manager
and Certified Health and Life Coach
Jennifer moved to Florida in 1992 from a small town in New Jersey as a young adult.
She obtained her bachelor's in Social Work at the University of West Florida. After graduation, she relocated to the Jacksonville area to work for the Department of Juvenile Justice; leading a level II program focused on prevention.
In 2000, Jennifer pursued her Master's in Social Work at Florida State University. After graduation, she entered the medical field and worked 18 years for a local Jacksonville hospital as a medical social worker. During her time there, she held a leadership position after completing the agency’s Emerging Leadership Program. Jennifer obtained her Lean Six Sigma Green Belt in Operations Performance Improvement (OPI). This opened up opportunities for her to be employed as a Performance Improvement Specialist with a local hospice agency.
Over the years Jennifer gained additional skills while employed by a local rehabilitation hospital, two home health care agencies, and partnering with surrounding area skilled nursing facilities. In addition, she provided educational marketing services to the northeast area physicians and senior living facilities concerning the benefits of hospice.
From this experience, Jennifer obtained her Health and Life Coaching certificate and has branched out to open her own business helping the senior community be empowered to make the best quality of life decisions for themselves and live life to the fullest: even during their end-of-life transition. The services are delivered in a geriatric care management style for the senior and supportive coaching for the caregiver.
